Nurture Pediatrics and Wellness, the pediatric medical practice of Dr. Alita Rice, will close on December 31, 2012. A letter has been sent to parents as well as an authorization to release copies of medical records. These may also be viewed by clicking on the links below.

Requesting medical records:

Through June 30, 2013, there are 2 options for obtaining records.
Option 1: Chart summary (consists of a list of medical problems, a list of medications, and a immunization record) - Please complete an authorization(click on link below) and mail with your $10 payment (check only) to Dr. Alita Rice, PO Box 270, Southfield, MI, 48037. Payment must be received before your request can be processed. Records will be mailed.
Option 2: Copies of medical records - Please complete an authorization (click on link below) indicating the date range and the records needed (specific types of records or all records).There is a per page fee for copies of records as noted on the authorization. Keep in mind that the  average chart contains 25-75 pages. Mail to Dr. Alita Rice, PO Box 270, Southfield, MI, 48037 or fax to 248-218-5801.  You will be notified of your total cost. Payment (check only, made out to Dr. Alita Rice) must be received before records can be copied. Records will be mailed.

Beginning July 1, 2013, there will only be 1 option for obtaining records. Copies of medical records will be available by completing an authorization (click on link below) indicating the date range and the records needed (specific types of records or all records). There is a per page fee for copies of records as noted on the authorization. Keep in mind that the average chart contains 25-75 pages. Mail to Stevens Records Management, 3663 Elizabeth Lake Road, Waterford, MI, 48328 or fax to 248-681-9754. You will be notified of your total cost. Payments to Stevens Records Management (check or cash) must be received before records can be copied. Records will be mailed.

Requests may take up to 14 days to process as records must be retrieved from storage.
